Medicare Redetermination Requirements are as follows:

  1. Copy of all pages of most recent checking account statement.

  2. Copy of Life Insurance Policy number, cash value, death benefit or cash value letter from insurance company.

  3. Copy of Health Plan card (both sides)

  4. Copy of all pages of three most   recent Direct Express Card Statements

  5. Last four paystubs or a letter on company letterhead with your gross monthly wages with your employer's signature. 

  6. Copy of Social Security award letter showing monthly benefit amount.

  7. Copy of red, white, and blue Medicare card, both sides.

  8. Copy of Social Security card, both sides.

  9. Copy of Driver's License or state identification card.

If you fall into this category, you might want to apply for help through Social Security by calling 1-800-772-1213 or going to www.ssa.gov

to sign yourself up for extra help with Prescription Drug cost. Once that is done, Social Security will automatically send your information to your state HHSC, aka, Medicaid, where you reside to sign you up for a Medicare Savings Program.

There is a great deal of confusion regarding where do I go to sign up for a Medicare Savings Program. You sign up at your local State Medicaid office, aka HEALTH AND HUMAN SERVICES COMMISSION! You can also call 211 and sign up over the phone. You simply request that you want to sign up for a MEDICARE SAVINGS PROGRAM. If you say that you want to sign up for Medicaid, you will be denied the benefits that you are entitled to receive. The FINANCIAL HELP that you are entitled to is through the Medicaid office, but the program is referred to as a MEDICARE SAVINGS PROGRAM. Please note: When filling out the application for yourself, please fill in the circle that refers to a MEDICARE SAVINGS PROGRAM. While you are at it, you might as well fill in the circle for the SNAP program as well. You will qualify to receive additional help to pay for food. There are HHSC/STATE MEDICAID locations in every county and city throughout the state of Texas. State Medicaid offices also go by the name of HHSC (Health and Human Services Commission) It's also where young people under 18 get their CHIP. To many individuals, HHSC/MEDICAID office is better known as where I sign up for my FOOD STAMPS. Yes, it's all at the same PLACE! You can also apply online at www.yourtexasbenefits.com.

Simply set up an online account and sign yourself up or find someone, whether

it be a family member, friend or foe, who is computer savvy enough, to sign you up for a Medicare Savings program!

However, this process can take weeks or even months to get approval. I, personally, recommend calling 211 or physically going to the actual Medicaid office where you can get signed up and approved all in the same day. Once you are approved, and you receive an award letter (see copy of award letter on the link at the top of this page) verifying that you have been enrolled in a MEDICARE SAVINGS PROGRAM: Please contact Senior Healthcare Professionals

There is much confusion regarding Medicare Saving Programs

There are 4 kinds of Medicare Savings Programs:

1. Qualified Medicare Beneficiary (QMB) Program—If you’re eligible, the QMB Program helps pay for Part A and/or Part B premiums. In addition, Medicare providers aren’t allowed to bill you for Medicare deductibles, coinsurance, and copayments when you get services and items Medicare covers, except outpatient prescription drugs.

Pharmacists may charge you up to a limited amount (no more than $3.70 in 2017) for prescription drugs covered by Medicare Part D. (If you have QMB, you automatically get Extra Help. See page 97.)

To make sure your provider knows you have QMB, show both your Medicare and Medicaid or QMB card each time you get care. If you get a bill for medical care Medicare covers, call your provider about

the charges. If you have Original Medicare, show your provider your Medicare Summary Notice (see page 63). It will show you have QMB and that you shouldn’t be billed. Tell them that you have QMB and can’t be charged for Medicare deductibles, coinsurance, and copayments.

If this doesn’t resolve the billing problem, call 1-800-MEDICARE. Medicare will ask the provider to stop improper billing, and refund any incorrect payments you made. If you’re in a Medicare Advantage Plan,

2. Specified Low-Income Medicare Beneficiary (SLMB) Program—Helps pay Part B premiums only.

3. Qualifying Individual (QI) Program—Helps pay Part B premiums only. You must apply each year for QI benefits and the applications are granted on a first-come first-served basis.

4. Qualified Disabled and Working Individuals (QDWI) Program— Helps pay Part A premiums only. You may qualify for this program if you have a disability and are working.

The names of these programs and how they work may vary by state.

How do I qualify?

In most cases, to qualify for a Medicare Savings Program, you must have

income and resources below a certain limit.

Many states figure your income and resources differently, so you should

check with your state to see if you qualify.

Medicaid

Medicaid is a joint federal and state program that helps pay medical

costs if you have limited income and/or resources and meet other

requirements. Some people qualify for both Medicare and Medicaid and

are called “dual eligibles.”

What does Medicaid cover?

■ If you have Medicare and full Medicaid coverage, most of your health

care costs are covered. You can get your Medicare coverage through

Original Medicare or a Medicare Advantage Plan (like an HMO or PPO).

Section 8: Get Help Paying Your Health & Prescription Drug Costs 101

■ If you have Medicare and/or full Medicaid coverage, Medicare covers

your Part D prescription drugs. Medicaid may still cover some drugs and

other care that Medicare doesn’t cover.

■ People with Medicaid may get coverage for services that Medicare may

not or may partially cover, like nursing home care, personal care, and

home- and community-based services.

How do I qualify?

■ Medicaid programs vary from state to state. They may also have different

names, like “Medical Assistance” or “Medi-Cal.”

■ Each state has different income and resource requirements.

■ Many states have expanded their Medicaid programs to cover more

people. Even if you were told you didn’t qualify for Medicaid in the past,

you may qualify under the new rules.

■ In some states, you may need to be enrolled in Medicare, if eligible, to get

Medicaid.

■ Call your Medicaid office for more information and to see if you

qualify. Visit Medicare.gov/contacts, or call 1-800-MEDICARE

(1-800-633-4227). TTY users can call 1-877-486-2048.

Demonstration plans for people who have both Medicare

and Medicaid

Medicare is working with some states and health plans to offer

demonstration plans for certain people who have both Medicare and

Medicaid, called Medicare-Medicaid Plans. If you’re interested in joining

a Medicare-Medicaid Plan, visit Medicare.gov/find-a-plan to see if one

is available in your area and if you qualify. Call your Medicaid office for

more information.
